Texas Medical Center Primary Office

Our office is located at Baylor Clinic Building:6620 Main Street, Suite 1510, Houston, TX 77030 ( in the corner of Main Street and Southgate Blvd).  Valet parking is available at the Main Street entrance, and the Self-Parking is located on Southgate.

This building combines a ten-storey parking garage and five-storey medical offices. It is connected to the O'Quinn Medical Tower at St. Luke's (formerly known as St. Luke's Medical Tower) by the skybridge to provide easy access to patients. Patients can simply follow the instruction sign located at the parking garage elevators and transfer to the medical office elevators on level 3 or level 9.

If you need to schedule an office appointment, please call our office at 713-795-4444 during normal business hours (Mon - Fri: 7:30 - 4:00).

PLEASE DO NOT SEND ANY EMAIL TO THE OFFICE IF THE PATIENTS REQUIRE IMMEDIATE ATTENTION.

After we schedule the office visit appointment, patient can review the financial policy on our Welcome Package section and download the New Patient Form. We also provide a cover page for your convenience when you fax all necessary forms to 713-795-5254 along with a copy of your picture identification card (e.g.driver license) and a copy of your insurance card (front and back).
