We accept variety of insurance plans, and due to the complexity of managed
care contracts, we suggest patients to verify our doctors’ participation
with their insurance company prior to make the appointment. Please use this matrix as
a reference only.
This list of our managed care contracs is always changing, so if your plan does not appear on our websie, it does not mean that we are unable to provide treatment. Likewise, we cannot guarantee certification by any of the plans listed on our website at the time of your visit.
Depending on your insurance plan, pre-authorization from your primary care physician may be required for some or all types of treattment. Obtaining pre-authorization before treatment is the responsibility of the patient, however, our staff will assist you with the process.
